One-time repair naming the prescribing doctor on a prescription created under the V1 process, which only ever carried the free-text issuedBy.

Valid in every status: such a record is normally already past receivePrescription and requestPrescriptionApproval, the only other mutations that set the field, and reverting its status to reach one of them would discard its approval. Refused once a doctor is named — the assignment is final. issuedBy is left untouched; it records what the paper prescription said.
